As some of you may or may not know, I had switched to a 1pc appliance quite a while ago, mostly to help lower costs. I was consistently getting at least 7 day wear before the edges began to peel more than I'd like, and not a single leak during that time. Most appliance changes were 10 days apart, and a few times it was up to 12 days. Then I went back to my typical 2pc, just to use up what I had leftover, and boom - leak after 3 days. With all other factors being the same, my thoughts are that the added surface area from the 1pc gives far more support to the appliance than the 2pc does - especially as the bag fills. Since 3, 4, 5 days were generally the "max" I would get with a 2pc, I think I'll be sticking with the 1pc. Has anyone else had this same experience between 1 and 2 pc appliances? I have only tried the 2 part disposable bags because originally the thought of emptying and cleaning the bag seemed distasteful. SS Has anyone else had this same experience between 1 and 2 pc appliances? I have had the opposite experience. But I am not a good case study on anything these days. I sometimes get a couple days on a two piece, but what I get in the one piece is measured in hours rather than days. It is getting harder and harder to not get discouraged by this. Nothing I am trying is helping. I have had 29 ostomy changes in April. 😢 -Liza I find that it's easier to put on a 2pc, and I have to use a different technique when putting on a one piece, but the longevity is noticeable. Do you have a list of the products you've tried? Perhaps put it in a separate thread, and we can look for commonalities among the ones that have failed. Just your friendly neighborhood ostomate. Eric, my wafer on my one piece is the same size as the wafer for my two piece. My issue is never the edges peeling back. It is always stool getting under the wafer and tunneling it’s way out. With the two piece, I can usually feel the burn in my skin in plenty of time after it breaches before it is a full blown leak. But with the one piece, twice it totally blew out, at work. After that many days, the adhesion may have been weakening so added pressure pushed through. No leaks while awake. Why? This is a very interesting thread to me as my new ostomate daughter just had her first leak overnight. Her bag does fill with A LOT of gas overnight. Could that be part of the problem? She had just changed her appliance yesterday morning. We are thinking we did something wrong in the fitting. I'm starting to use a 1-piece depending on the situation. I've had my stoma since end 2019 and during COVID times been working from home close to 100% and using a 2 piece Hollister. Now that I'm occasionally going back into the office, I'm more conscience of a 2-piece system sticking out and a 1-piece is sits closer to the body. Wear-time over 4-5 days makes me nervous and reading you get 7-10 days, I may try for a bit longer. I know it's unnecessary and I shouldn't, but I always use a Eaking seal, 2pc or 1pc. It prevents me from having to cut the wafer to size. I mould the seal to close around the stoma and use a precut wafer. Her bag does fill with A LOT of gas overnight. Could that be part of the problem? Yes, absolutely. Gas tends to put a lot of pressure on the opening (hole) that the stoma comes out of, and that's the only weak point for leaks to happen. Gas should settle with time, but it can still depend highly on the foods consumed (food diary is recommended). If gas is a consistent problem overnight, it may be helpful to have her set an alarm in the middle of the night, simply to empty the bag. Nighttime leaks are so disruptive :( have only had two leaks. Even with the wear time I get with a 1pc, the center of the wafer is still strong, but everything past the center has essentially no adhesiveness whatsoever. Just your friendly neighborhood ostomate. Since getting the ileostomy I have almost no pain or emergency bathroom use issues due to the food I eat. But as I noted, gas is now an issue when sleeping. In my case there are many dairy products that will cause gas and fast transit time with a lot of liquid. But one cannot live without cheese. So eat it long before bedtime. Also in my case I cannot live without Rum and Coke. Carbonated beverages contain gas. What doesn't bubble out in the glass, will bubble out in your guts. A lot of folks drink a lot of carbonated beverages. These also need to be timed out before bed time. But I am still sipping a Havana Club and Coke at almost 23:30. Cuban roulette. Why? @kedikat I use generic gas relief gell caps (Walmart's Equate Gas Relief) and they do a very good job of preventing it. I eat a LOT of leafy greens, onions, garlic, etc that are huge gas producers otherwise. Interesting I was using a single piece Convatec appliance 22771 and I was lucky to get 3 days of use from it. I have switched to a two piece mouldable piece appliance the convatec 411805 57mm flange and the 416421 bag also Convatec normally i get 5-6 day use out of it before the flange expires which amazing for me! Mind you earlier this week I had a flange explode twice ! Within 24 hrs mostly however because t’of the long drive and I think the seat belt wanting to always slip onto it was the culprit! I love the foldable Velcro on this appliance and foldable flap! I got to try it through the ostomy clinic. As for me i had an good 3 week streak Until I woke up one morning swimming lol. My fault for making deer sausage and sauerkraut late one night as a snack. So let that be a warning to you all DO NOT GIVE IN to snacking before bed time 🥱 Question for those of you getting more than one day wear from your appliance….. how? Are you emptying ? Are you rinsing? Do you use Ostomy deodorant ? What is your secret because I can’t get past one day with any bag. As soon as the shower it’s over. It seems soap and shampoo gets down in there no matter how careful and then I start getting that itchy feeling around my stoma like the bag is going bad. @chelly ... Good question!! But to be fair to you... you should state the Type of Stoma you have, ileostomy, colostomy, urostomy, the Pouch you are wearing and your changing routine listing products you use as well. This way you will get answers from those wearing the same pouch as you. It wouldn't hurt to say how much bending you do in a day either. I have noticed with my Hollister 8331 one piece pouch that the more hard housework I do the less wear time I get bcuz I think the heavier my work & the hotter I get. There are many factors to wear time. Body heat, swetting, use of tummy muscles, do you have a trim tummy or a bit of a cushy one I think matters too. Add to that, is your stoma flush or does it protrude a good inch to inch & a half like mine does. I've learned over the years that those with flush stomas seem to have more frequent leaks. It wouldn't hurt to state the type of work you do on the job either. How much skin barrier I've used has made a difference in my wear time also. Where is your clothing waiste line sitting at, under, over or on your stoma. I think even enviromental temperature can play a roll as well. When I first had my ostomy I had issues with leaks galore then once I stumbled on VO someone on another forum said they were using a ring around their stoma with an ileostomy & once they stopped using it their leaks stopped too. So I tried it & presto, my frequent leaks stopped. Plus I realized my output was pooling around my stoma at night so I raised the head of my bed enough to let it flow to the bottom of my pouch. Then I started getting less leaks at night too. So... many factors matter here. I hope you can get the answers you need. Linda @dlkfiretruck it’s an ileostomy. Currently using sensura mio deep convex. Changing routine is daily due to it will not hold up past 1 day. At changes I slowly remove the pouch as it peels quite easily and does not rip the skin. Then using remove adhesive remover around the area and sometimes have to use alcohol after the remover , if you recall I have that sticky issue. Then I use wet paper towels with plain water to remove the excess. I go get into the shower and rinse that area further with my handheld shower head. I proceed to wash face and hair bring very careful not to let anything slippery get into that area. I refrain from conditioner and will only use it once in a while. Then I do the upper body using a clarifying soap which will remove build up. Then I move to the Ostomy area using the clarifying soap and then do a second cleaning with dial white. Lower body is last basically same manner as top half. @chelly I change my wafer and bag twice a week. Monday and Friday, which has me using my bag and wafer for 4 days and then 3 days. I use Hollister convex wafers and drainable bags. Prep involves a quality barrier spray in 2 coats, drying between coats, then placing the wafer over my flush stoma. Next important step, I hold first the inside the flange area, then the entire wafer with moderate pressure for about 5 minutes each while laying down. This really sticks it to my flesh well. I'm very active in farming and tending my livestock, and as I do everything from a wheelchair, it's constant bending at the waist to reach the ground so the area around my wafer gets tons of movement. Bag clean-outs are done sitting slightly sideways on the potty and directing my bidet up into the neck of the bag. 5 seconds and it's completely cleaned out and I'm back to work! I haven't had a leaking problem in several years. Edit: Mine is a mid transverse colostomy Retired engineer, now goatherd @chelly Two things that I always include in my change routine that I didn’t see in yours. First, when I am in the shower with no appliance on, I use a small nail brush to GENTLY scrub and exfoliate the area around my stoma. I find that if I don’t do this, my flange tends to loosen around the edges quicker. Second, I warm up the flange with a hair dryer before I put it on my skin. Then I hold it in place for a few minutes. If you try using a hair dryer, just be careful that it isn’t on a really hot setting or you can cause yourself some discomfort. I don’t know if these steps would be helpful for you, but they make a difference for me. Laurie Just a semicolon @goatherder that’s great!! Twice a week for me would even be good! I’m wondering in my case is if it could be that I have to shower twice a day. Once before work and then once when I get home? My jobs are nasty so I always shower when I get home. Work anywhere from 4-5 days a week so that’s a lot of showers and bags. You may have something there about laying on the bed for a while and pushing on the wafer part around the stoma. I may have to give my self extra time. Maybe I’m rushing too much. @tigerlily I’m very interested in the mail brush step. Do you scrub using the nail brush with soap or is that before or after you soap up? Also what soap do you use? The hair dryer I have done before but my Ostomy nurse and the pouch manufacturer tells me not to do that with extended wear bags. I have a sticky melty issue. This might help

then I go into my other restroom and I wait till the area is dry and I apply the pouch. I press in it with my hand for a minute or two and make sure everything has sealed well.
I can’t use barrier wipes or rings any longer due to the overly sticky issues.
note than even with other pouches I have used and tried, changing daily still had to be done for the same reason.
I have been a person that sweats easily in humidity especially in summer.
the type of work I do is easy but I do have to clean the clients homes. I get what you’re saying about the more you move around it will affect how it stays on as that happens with me too!
my cloths are leggings that fit tight but not to tight. The Ostomy nurse told me those are perfect because they are like a support to everything.
but as I was saying about pouches it is all of them. I have probably tried every pouch out there but now with the sticky issue I have to stay with mio because it’s the adhesive that’s most agreeing with me these days.

i got to hand it to you, doing all that hard labor with your Ostomy and your still able to change only twice a week. That’s amazing!
