Hi Rick, here's what the Comfortbelt website says regarding measurements: Use the below table to figure out what is the best size for you. Measure your waist size in inches just underneath the stoma and select the corresponding belt size (numbers 1 through 10). They also have a video on the page: https://www.comfortbelt.com/products/mens-comfortbelt The flange opening is universal on this product.
