ADP Ostomy Grant Renewal

To anyone here who has or is receiving this, I hope someone can provide some feedback. My ostomy grant expired in October, and I'm up for renewal. Long story short, I've been waiting 8 long months to see a specialist...a colorectal surgeon, and I finally have my appointment with her tomorrow. However, I have no idea if she will be willing to sign this form to get my grant renewed, as she is not who performed my ileostomy surgery. As it stands now, I have no family doctor, as I relieved him of his duties back in March, and as for the surgeon who performed my ostomy surgery, she is ancient history as well, and I will never go back to her for any reason. That being said, has anyone run into difficulty with having a doctor sign this form so the grant can be renewed? If so, what avenue did you pursue? Thanks :)

I would see if a walk in clinic could help, since you did have the grant already. Are you able to find a new family doctor?

Hi Eric. I hope she will just sign the form without any hassle. To be honest, I called ADP last week, inquiring about why I need to be renewed, as when I originally applied, my surgeon wrote permanent. I'm not sure what part of that word ADP doesn't understand. I guess I must be living in a parallel universe where that word means something different. As for finding a new family doctor, no. Trying to find new family doctor is next to impossible, unless I want someone right our of med school, and that is not for me. I posted in the forums lastnight about this very subject.

I've had letters asking me to confirm that my "condition hasn't changed" (i.e. I still have a PERMANENT ostomy) and I just let them know.  But I don't recall having to go back to get paperwork signed. 

It's entirely possible that it wasn't filed correctly - I even hear some people saying their ostomy grant "expires" after a few years before it needs to be renewed, but mine essentially has no end date.

That's odd yours doesn't have an expiry date...mine is every 2 years. I called ADP last week about this, and the person I spoke to said "permanent means every 2 years". Nice they are taking your word for it...for me, they need verification from a doctor. Oh, and a walk-in-clinic told me no. I'm seriously considering just giving up on this. It's too much stress, run around and nonsense for me to have to "prove" something that was clearly noted as permanent 2 years ago when I applied for it.

I posted a copy of my approval here:  https://www.veganostomy.ca/canadian-disability-tax-credit/   It says "and future years", rather than an end date as I've seen before. I do have to confirm every two years that I still have an ostomy, but I don't have to reapply or anything like that.

It was a process to get started, but once you're approved it should be easy after that. I still think that they filed your application wrong for you to have to go through this much trouble.
