Stoma Shape

Hello,

My stoma is uneven part concave and part convex which makes it difficult to keep the skin under the stoma covered and protected.  I tried using barrier rings but it does not adhere to the convex section leaving the skin unprotected.  Although I have not had any skin irritation so far, I worry that it could develop.  Does anyone have any suggestions or appliances that resolve the issue?

Hey Fred. 

Are you using a convex appliance? Have you tried stoma paste? If so, how was your experience?

 I have kind of an innie stoma and found that going to the Coloplast deep convex wafer and using the 2mm thick mouldable barrier ring by Coloplast that it seems to cover and protect the skin under the stoma. Since using these two products, I have had far fewer leaks than with the other appliances I tested. Hope this helps.

Yes, the convex wafers really do help with an 'innie'. My stoma telescopes ( as I just posted somewhere else in this forum). It goes in and out. I had a lot of leaking...very bad, depressing, I know. I use Hollister now.

I also use a barrier ring with the wafer . And I use stoma powder just at the base of the stoma.

One of the best things I discovered was to use a hair dryer. I change after I shower ( naked as Eric says), then use the portable dryer to dry and HEAT the skin AND the wafer & barrier ring. It seems to stick much better if its dry and warm.

I apply the ring to the wafer and THEN  the whole thing to my skin.  Then more heat & pressure.

I don't know if that helps, but it works for me.

I can see how the process ( if it works) can become ritual!

My stoma seems similar to yours. It's oval shaped with the top being just slightly above the skin level, the bottom sunk below, and as Dona says, it telescopes. I can't use convex appliances as they cause many issues for me, and do not give me any benefit that convexity should. After a lot of trial and error, I have found a solution that works for me.

I use a Hollister flat wafer and Coloplast Brava Strip Paste. Since convexity is an issue for me, I use the strip paste only on the bottom half, and I press it into the thinnest I possibly can. This works very well for me and I almost never have leaks.

My biggest problem now is finding a product that does not erode too much. I am currently testing the new Brava Protective Ring (Thin). I've only had it on for 24 hours but so far it is very comfortable, and minimal erosion. 

Don't be afraid to experiment, or to call the manufacturer and ask for samples. They are usually happy to send those samples out.

These all sound like good suggestions.  Seems that sometimes the barrier rings adhere to the skin and sometimes not.  I will try your suggestions. Thanks.

I find that barrier rings can differ considerably depending on the brand. And they need a dry surface to stick to.

Hi Kae,

My situation seems to be very similar to yours and Dona's  telescoping and the shape, etc. I use the Holister barrier rings that were working well until the last few weeks.  I think my stoma has changed shape and I need to adapt and will give your suggestions a try. Thank you.

Hi Fred,

I used the Hollister rings for quite awhile. Then my supplier switched some of the Hollister product for the Coloplast ( brava) moldable rings. I found them ( the coloplast) to be much better. Longer lasting and not so prone to moisture . Softening etc. I am a swimmer ( back yard) and was not able to get in the pool for years before my illiostomy. So I am back in the water and looking for things that last even tough they get soaked..  The Shield ( suppliers) no longer can get me the Coloplast product, so I have asked for some samples of bags,wafers from Coloplast to see how it goes.  They are sending some wafers that have a moldable center. I think Eric has maybe reviewed them here?  I will share what ( if anything) I learn.

Sounds like I'm pretty lucky to get a small 24mm circular stoma but I still use a convex base plate as I have ileostomy but as I'm on the chubby side I have to use half a convex barrier seal (on bottom half) as will as a slim barrier seal (top half) and since using that I haven't had leaks YaY

If you are referring to the new Protective Rings by Coloplast then, yes, I have reviewed those! HERE

Everyone I know who's tried them say they work very well.

Those are the ones. I'll see how it held up when I change tomorrow. Doesn't seem to swell or erode like others, and seems to have adhered better too.
