Tips when applying for the Disability Tax Credit (Canada)

There were a few questions by @dlkfiretruck that I'd like to answer:

Eric...I have a question regarding the DTC Certification...also I am not sure how I missed out on being included in this when it was happening.   There does not seem to be a lot of space for the doctor to write down the things you suggested we make note of.  Should the doctor make comment about the length of time and duration, time it takes when travelling, leaks and blockages time and healing....my numerous hospital stays and so forth.  How did your doctor do this?  I made notes of your points t take to the office when he fills it out.  I am also in the middle of the week in tracking what time and how long it takes to care for the requirements of the Ostomy.   Can he and should he include another paper with this information on it?  Thank you for your help.

I would definitely have your doctor write as much as possible about how an ostomy affects your daily living - including time to irrigate, how often you wake up to empty your appliance, planning that needed when going out, etc. 

If they need to use another sheet to write the details, then that's great!

I don't know if hospital stays would specifically need to be noted, but if you've had consistent problems that have landed you in the hospital (i.e. blockages, hernias, etc.) then I would have the doctor list that as well.
